Vancomycin Tool

INSTRUCTIONS - IT IS ESSENTIAL THAT THE TWO LEVELS WERE TAKEN WITHOUT ANY VANCOMYCIN GIVEN BETWEEN THEM:

  1. If you wish to dose for a target other than AUC:MIC of 400 you can change this in panel D.
  2. Enter the current dose and dosing interval (every X hours) in panel A as well as the date and time of the dose prior to levels
  3. Enter the peak and trough concentrations (or any 2 concentrations without doses between them) with the corresponding dates/times
  4. Select (if necessary based on panel B and any changes in renal function) a new dose from one of the green regimens listed.
    • The dark green is chosen to maximize interval and minimize dose. This may not always be what you want.
  5. After a dose adjustment you should check new peak/trough (usually after 3rd/before 4th dose)
  6. If no adjustment was needed; recheck Cr every 1-3 days depending on risk (or presence) of renal failure and trough level every 2-5 days until completion of therapy or discharge
  7. If follow up trough is within 25% of expected steady state (above) and creatinine is stable the AUC is most likely stable
  8. If follow up trough is outside of 25% of expected or renal function is changing draw additional levels and recalculate AUC
