Changelog:
Version 1.23
- Adjusted initial Vd to use 0.8L/kg in underweight and normal weight patients and 0.7kg/L (using adjusted body weight)
- Adjusted Clearance - Now using method proposed by Fewel in place of Bauer (but leave Bauer for comparison)
Version 1.22
- Now available to general public
Version 1.21
- Fixed bug with entry of dates and times. Will now only accept valid dates/times and some basic error checking
- Added a cell which turns red if there appears to be an error with the dates/times despite the data entry restrictions
- Changed initial dosing volume of distribution in patients above ideal body weight. Previously had used 0.7*(IBW + 0.5(WT-IBW) but now it is using 0.75*ABW.
- Modified creatinine clearance formula -- now using adjusted body weight if weight >15% above IBW. Previously had used the Salazar-Corcoran equation.
Version 1.2
- 2-level adjustment now based on dates and times as opposed to user manually calculated number of hours
- Allow troughs to be slightly below 10 (although generally not preferred due to concern for resistance selection, it allows for some necessary flexibility)
Version 1.1
- Optimization of automatic dose selection to account for target AUC 600-800
- Bugfixes for identification of off target values
- Separation to separate pages for ease of use
Version 1.0
- Initial release of initial dose calculator and 2 level adjustment
